1. Customers with Rogers Home Phone and long distance plan, whose accounts are in good standing and whose service is billed on the appropriate platform as identifi ed by Rogers, will not be billed for long distance charges. Standard per-minute and other wireless plan charges apply.
2. The 500 long distance minutes may not be carried over and must be used in the current month. Calls made through a toll-free service, calling card service, dial-around service and operator assisted calls are not included.
3. A $5.95 monthly System Access Fee (non-government fee) applies in addition to the monthly service fees. Monthly 911 Emergency and Hearing Impaired Assistance fees and a monthly recurring fee for some long distance plans are extra. An early cancellation fee will apply to customers who terminate service within 12 months.
4. Price includes a monthly modem rental fee of $3.00. A one-time Activation Fee of $4.95 applies. Installation charges and a courier fee of $9.95 may apply.
5. Customers on 12, 24 and 36 month service agreements will receive 1 month, 2 months and 3 months unlimited local calls respectively.
6. Unlimited local calls, text, picture and video messages applicable to the 5 phone numbers designated on the MY5 list. Unlimited Canadian Long Distance calling is included in the $35 and $40 MY5 plans and applies only to calls initiated or received from your Rogers Wireless phone anywhere in Canada to and from your MY5 list. Long distance and roaming charges are extra where applicable, text to landline is not included and will be charged. MY5 terms of service: Only 10-digit Canadian-based phone numbers are eligible for the MY5 service. Customers’ own Rogers Wireless phone number, voice mail retrieval number and special numbers such as 1-800/1-900 are not accepted. One MY5 update per calendar month is allowed via rogers.com or Rogers Customer Care or on select phones. No credit will be applied for entered incorrect numbers. Other conditions may apply; visit rogers.com/my5 for details. Customers who subscribe before May 31, 2008 will receive unlimited picture and video messaging to any number until May 31, 2008. After May 31, 2008 customers who subscribe will receive unlimited picture and video messaging to their list of MY5 numbers only and any picture and video messages sent to numbers outside their MY5 list will be charged at the applicable rates.
7. A $6.95 monthly System Access Fee (non-government fee), a 50¢ monthly 911 Emergency Access Fee (non-government fee) and a one-time $35 Activation Fee apply in addition to the monthly service fee. Local airtime, long distance and roaming charges, additional service options selected and applicable taxes are extra and are also billed monthly. An Early Cancellation Fee (ECF) applies if, for
any reason, your service is terminated prior to the end of the service agreement. The ECF is the greater of (i) $100 or (ii) $20 per month remaining in the service agreement, to a maximum of $400 (plus applicable taxes), and applies for each line in the plan that is terminated.
